The Survey/Forms module can make surveys used to collect information from your constituents or from anonymous responders. They can also be for internal use only. Examples include:

Surveys can be sent via email, they can be internal-only, or they can be shared with a custom URL and QR code.
Polls, which are limited to one multiple choice question, can also be sent via email or can be shared with a custom URL or QR Code. One-question polls are not available for internal-only. Once a poll respondent submits their response, they will see a graph showing the total responses so far.
You have the option to connect the response provided to a constituent record by collecting a name and email address. Or, you can make your responses completely anonymous not collecting any specific information about the constituent.
Your surveys can also include logic if desired, to allow you to customize the flow of the questions and direct the respondent to the appropriate next question based on a response.
