What do I do on the Survey Info step of the Survey Wizard?

On the Survey Info tab, you must provide a name for the survey or poll.
If this will be accessible only from within the software, you can check the box next to Internal Only.
Optionally, you can also enter tags or a description for the survey to make it easier to find in the future.
If you are planning to distribute the survey/poll via email, you will need to enter the name that the email will appear from, the email address that it will be sent from, as well as the subject line of the email.
