Yes, within the setup of the online Donation Form you are able to turn on the option to gather Tribute information for the Donation. On the Donation Page Settings tab scroll down to Additional Options and find the "Show Tribute" setting. Once this setting is turned on you will be able to select which fields should be visible on the form. Check the box to include the question on the form, uncheck the box to hide it. The fields available are as follows:


Tribute Notification Name - Who should be notified about this tribute donation.

Tribute Notification Informal Salutation - How should that person be addressed during notification.

Tribute Notification Email - What email address should they be notified at.

Tribute Notification Address - What mailing address should they be notified at.

Tribute Notification Notes - Any additional details about the tribute or who should be notified.
